You must provide the following information at the time of registration to enroll a student in a Trussville City School. All enrollments are temporary until the information presented is verified and deemed acceptable and complete. These requirements are for new and returning students.
• Proof of legal custody if you are not the parent or proof of custody in the case of divorce. (Notarized statements are not accepted)
• Proof of residence including such items as a deed to a current residence in Trussville; bona fide lease in the name of the parent or legal custodian and signed by the landlord and tenant (deed or lease required); two current utility bills (cell phones are not acceptable utilities) in the name of the parent or legal custodian at that address (Alabama Power bill required). We are not able to take a disconnect notice.
• A current Alabama Immunization Form for the student from a physician or the health department (form must have a valid expiration date) - if not on file at the school.
• Original Social Security Card for the student in the student’s name – if not on file at the school.
• Original birth certificate for any student not previously enrolled in a Trussville City School.
• The withdrawal form with grades and a current transcript (last current report card) from the previous school attended, if applicable. The complete address of the previous school attended must be included in order to request records from that school
• A signed certificate of residency
• A copy of the legal guardians driver’s license or other state photo identification card
Trussville City Board of Education reserves the right to accept or reject any documentation provided. Call Support Services to confirm the school zone for any address in question 228-3031.
