THIS IS FOR STUDENTS WITH BC/BS PEEHIP ONLY
The Public Education Employees’ Health Insurance Program (PEEHIP), is happy to announce a new benefit being offered this fall. In cooperation with the Alabama Department of Public Health’s Wellness Program, PEEHIP will be offering the influenza vaccine in schools to children who are covered under Blue Cross Blue Shield (BC/BS) PEEHIP cards with the letters EDU. For children not covered under BC/BS PEEHIP, influenza vaccine will be administered by a separate company through the “Teach Flu a Lesson” program instead of through the PEEHIP program. This year, the PEEHIP program will be offered to 24 schools, and we hope to expand statewide next year.
Parents must complete both the Influenza Consent Form for Children and the Vaccine Administration Form, ADPH-IMM-66/Rev. 10-11, and return them to the Wellness Program at the address shown below:
